Commmercial Contractor Application: Proof of insurances not required. Call May x259 for fees . If you don't know what the classification is enter a description of the work and the authority will determine as it could be 2 classifications and 2 fees. Print the application on 11 x 14 inch paper (legal size) don't need original. Note that there are multiple forms: (App, Financial Statement, Affidavit, Work Experience, and QP form). The exam is scheduled 6 weeks, not 3 week after receipt of application. The license is not issued until after the qualifier takes and passes the exam.The licensing board meets monthly ( 3rd Thursday of the month). The passing scores must be received in the office at least 10 days before the board meeting. There is a 60 day waiting period from the time the application is received in the office for out of state applicants before the license is issued.
